Health care and sound financial planning are linked. At The Spine Institute Center for Spinal Restoration, we believe you should be informed when making decisions about your health and want to make sure you understand the benefits available to you under your insurance before entering treatment.

The Spine Institute Center for Spinal Restoration is a provider of some insurance plans, and we accept both Worker’s Compensation and Medicare. While The Spine Institute physicians are generally part of the same plans, this is not always the case. To ensure that your selected physician accepts your individual insurance, contact your insurance carrier directly by calling the customer service telephone number located on the back of your insurance card. Your personal agent can help you determine your insurance benefits and eligibility.

Please be sure to bring your current insurance card with you during each visit to The Spine Institute. We also ask that you inform us of any changes in your coverage or home address so that you are able to full advantage of your insurance coverage.

Worker’s Comp for Back & Neck Treatment

Back pain is one of the most common reasons for missed work in the United States. If you have been injured on the job, you may be eligible to have your treatment paid for by your employer.

Prior to scheduling an appointment, you must first obtain approval for your adjuster or case manager and will need the following information:

  • Your employers name and address
  • Workers’ compensation claim number
  • Insurance carriers name and address
  • Insurance adjusters name, phone and fax numbers
  • Date of work related injury

Medicare for Spinal Treatment & Surgery

As a participating Medicare provider, we are able to help you reduce out-of-pocket expenses and can assist you with the billing process. All you need to do is bring your Medicare card and supplemental insurance information with you to your visit and we’ll take care of the rest.

Self Pay for Medical & Non-Medical Procedures

Depending on your coverage, you may be responsible for the payment of a portion of your services and treatments. Patients are most commonly responsible for the office visit charge, as well as diagnostic or therapeutic tests performed during the visit.

If your individual insurance or workers’ compensation claim is not accepted by your insurance provider, you will be held responsible for the payment of all services. Please verify our acceptance of your insurance before making an appointment as changes may occur in the plans that we accept.
